Liability Waiver
I, the parent/guardian of the registered player, understand that hockey is a physical sport with inherent risks of injury, including but not limited to cuts, bruises, fractures, and concussions. I voluntarily assume all such risks on behalf of the player. I hereby release Petersen Hockey Development, its coaches, staff, and the hosting facility (NFP Canlan York) from any and all liability for injuries, accidents, or loss, theft, or damage of personal property that may occur during camp activities.
I confirm the player is in good physical health, is physically able to participate in a high-intensity hockey camp, and will bring and wear full approved protective equipment at all times while on the ice. I authorize camp staff to seek and consent to medical attention and emergency treatment for the player if I cannot be reached.
Media Release: I grant Petersen Hockey Development full permission to photograph and record video of the player during camp, and to use those photos and videos — including footage of on-ice instruction, drills, and games — for promotional purposes such as social media, the camp website, advertising, and highlight reels, without compensation. I understand these materials remain the property of Petersen Hockey Development.
I understand the player is expected to conduct themselves respectfully toward coaches, staff, and other participants, and that Petersen Hockey Development reserves the right to dismiss any participant whose behaviour endangers or disrupts the camp, without refund. I also understand the registration fee is non-refundable once camp begins.
